License Information

The Oregon Board of Medical Examiners  licenses physicians, podiatrists, physician assistants and acupuncturists. Information on individual physicians and other health care practitioners can be obtained from the Board either through an Online Search of Board Information or by telephoning the Board at (503) 229-5027. Available information includes:

  • date first licensed in Oregon;
  • license expiration date;
  • license status;
  • whether or not a licensee is in good standing with the Board;
  • address and telephone (address not available online);
  • self-reported specialty (which is not independently verified by the Board);
  • date of birth;
  • education. This information is available online but not over the telephone. However, it is included in a printed License Verification Report which can be obtained for a fee. The fee is $10 for each health care practitioner.

Information on up to 3 health care practitioners can be requested without charge over the telephone.

For a $10 fee, the Board will provide a printed signed License Verification Report on a health care practitioner. The report should be requested in writing addressed to the Board. The fee must accompany the request for the report. Click Here for information about obtaining a Verification Report. 


Malpractice Information.

For a $10 fee, the Board will provide a printed report on closed malpractice claims in Oregon against a physician or podiatrist, including lawsuits which have been settled. The information available includes the date and results of the lawsuit. Information on pending lawsuits is not provided. If there are no closed malpractice lawsuits, the report will indicate this. A malpractice report must be requested in writing and the request must be accompanied by the $10 fee. Click HERE for the information about obtaining a malpractice report.

The statute governing the disclosure of malpractice information is ORS section 742.400 which is available by clicking HERE.


State Disciplinary Actions.

For a $10 fee, the Board will provide a printed Disciplinary Report of a final board disciplinary action against a physician or other health care practitioner. The final resolution of the case and any board order is publicly available. However, the specifics of the investigation are not. Information on pending claims is not available.
